Presenting Problems

Presenting problem represents the identified problem that appears to have caused the client to seek services.  Clients may have up to three presenting problems, however, only the primary presenting problem is represented here.


SEXUAL ASSAULT

  • This Needs To Be Changed  This is reported when the client and the counselor, clinician, etc. are in agreement that the treatment plan has been completed and services are no longer necessary at this agency.

ALCOHOL RELATED

  • This Needs To Be Changed  This is reported when the client has completed the court commitment under which he/she was admitted and is no longer legally required to remain in treatment.

DRUG RELATED

  • This Needs To Be Changed  This is reported when the client leaves treatment against the advice of the counselor, clinician, etc. If the client and the counselor, clinician, etc. have not agreed that services are no longer necessary and the client has not been seen in 90 days, this discharge type is used. This transaction code is for outpatient services only.

ABUSE VICTIM

  • This Needs To Be Changed  This is reported when the client moves his/her residence to a different geographical location and it is no longer feasible to receive services at the present agency given the distance.

DEVELOPMENTAL DISORDER

  • This Needs To Be Changed  This is reported when the client transfers to another treatment agency regardless of whether it is funded by DMHSAS. A discharge is not submitted if the client is expected to return, e.g., from a hospital back to a community mental health center, and continuity of care needs to be maintained.
     

MOOD DISORDER

  • This Needs To Be Changed  This is reported when the client’s treatment is terminated to return to a correctional facility, such as jail or prison.

OTHER NON-PSYCHOTIC

  • This Needs To Be Changed  This is reported when the client was discharged due to breaking the rules of the facility. Client must have broken a written rule, e.g., showed up intoxicated, not just treatment non-compliant.

OTHER PSYCHOTIC

  • This Needs To Be Changed  This is reported when the client leaves an inpatient, detoxification, or residential treatment program or facility prior to the treatment plan, goals and objectives, or the prescribed period of time indicated by the program criteria has been completed. In other words, an AWOL indicates the client left the facility prior to the completion of treatment and the agency believes further services are still needed by the client.

SOCIAL CONDITIONS

  • This Needs To Be Changed  This is reported when the agency learns the client is deceased.

UNKNOWN / OTHER

  • This Needs To Be Changed  This is reported when a client has been formally admitted to a facility, but the client did not receive any services.

DEFERRED / NO DIAGNOSIS

  • This Needs To Be Changed  This type of discharge is executed when DMHSAS ceases to have a contractual agreement with a facility and all clients are administratively removed from the data system.
